allows for an easy dropdown option as the theme i use in shopify does not allow for dropdowns. I wish there was a way to specify pricing per option though.

This app what exactly what I was looking for. A simple way to add custom forms for different Shopify products. It has all the custom options you should need in a simple to use interface. The customer service it outstanding. I gave them access to my admin and they were able to add in some functionality for my theme within two business days.

It's simple and works great. I needed some help and they helped me big time. Thanks for assistance. Definitely I recommend:)

Super easy, added a Date-of-Birth field that is required for courses we sell. They even logged into my account (with permission) and matched the formatting.

Great for our online pet store, as we cannot sell FDA products to non-patients. The best part about this app is the technical support! I got my issues resolved in a very timely and professional manner! Would definitely use other apps from this company.

I love how easy it is to set up and customize. You don't have to be a programming genius to set it up. The app works right out of the box, and it takes less than 2 minutes to set up custom forms for each product. All you have to do is tag your products, use those tags in the app, and boom - done! =)

Seriously this is one of the best apps that I have installed! It allows me to set up multiple options on sales and allows my customer to choose what they want. Whenever I have a buy one get one promo...or buy any two products......the option to choose is now available.
