I searched high and low for a good simple "Contact Us" form app that I could use on my store. I've tried at least five different form builders and so far this was certainly easy to set up and has worked the best. For some reason with others, many of our customers have had error warnings and issues submitting. I had one small snag with installation and Danielle helped me out very quickly! I highly, highly recommend it.

Wow, Daniel, Dzung & Duke in Tech & Customer Support are really on to it. Very patient & very fast with great focussed follow through over several things that I needed help with on my first 2 custom forms. This is a very neat form builder, I'm glad I chose it, more features than a lot of the others I looked at. I need to get measurements from my customers and product use case so not being limited to 6 input fields like a lot of the apps I looked at was one of the winning points for me. Very generous with features & integrations. They let me know there was some jquery issue and overlapping elements with another app I use all the time and fixed everything really quickly.
